About You

The Census Management Coordinator should have the following qualifications:

  • Education: High school diploma required; A.S. or A.A.S. degree in a business or health-related discipline from an accredited college or university, or an equivalent combination of experience, education, and training.
  • Certification: Medical certification in medical assisting, etc. preferred
  • Experience: Minimum of 2 years’ experience in a hospital/medical office/managed care plan environment is preferred; Prior experience in a healthcare setting is required. Former experience as: Medical Assistant, office manager, medical secretary preferred
  • Skills: Excellent interpersonal skills. Previous customer service experience required; Strong follow up skills required, as well as the ability to manage multiple priorities require
  • Communication: Excellent verbal and written communication skills. Speaks, reads, and writes English
  • Technology: Working knowledge of Microsoft Office (word/excel) and other appropriate computer software, electronic medical records required. Typing, filing, and accurate data entry skills.
  • Relationships: Ability to build and effectively manage relationships with business leaders and external constituents; and,
  • Culture: Good judgement, impeccable ethics, and a strong team player; desire to succeed and grow in a fast-paced, demanding, and entrepreneurial Company.

About the Job

The Census Management Coordinator reports to the Director Clinical Operations/Care Coordination with accountability for effectively extending our care team approach by collaborating with hospitals, skilled nursing, and rehabilitation facilities to address inpatient care, discharge plans, and coordinate post discharge follow up on medical, social, and behavioral health needs. The Census Management Coordinator is accountable for providing strategy, judgment, organization, and evidenced-based analysis to influence decisions, and directly to meet CINQCARE requirements. They should embody CINQCARE’s core values, including, Trusted, Empathetic, Committed, Humble, Creative and Community-Minded.

Responsibilities

  • The Census Management Coordinator will have the following responsibilities:
  • Facilitates patient transitions through the health care system by retrieving, and providing, information that completes facility census.
  • Conducts daily retrieval and review of hospital/SNF/facility inpatient and emergency room census via portals, health information exchanges, EMRs, direct contact with facilities, etc.
  • Complete the census data by retrieving missing information i.e. reason for admission, review of medical history, anticipated length of stay and discharge needs. Follows up with facilities and maintains expected TOC/TFU timeframes as per workflow.
  • Provide continuous review updates concurrently during the hospitalization and post discharge episodes of care.
  • Participate in Daily Hospital Census Rounds forum to discuss the patient’s inpatient status, anticipated discharge and provide information that contributes to a post discharge plan in conjunction with the PHN, Nurse Case Manager and PCP, leveraging others in the Care Team as required.
  • Serves as the liaison between the CINQCARE TOC/TFU teams, PODs, and the healthcare delivery systems teams
  • Also provides administrative and customer service support.
  • Understands and records anticipated post-discharge care needs to ensure a smooth transition home or to other level of care setting.
  • Obtains hospital Discharge Summary, Medication List, and other pertinent information; shares with CINQCARE TOC/POD teams to line up TOC process.
  • Works closely with the CINQCARE/POD teams to monitor and track family member readmissions and frequent flier/High risk status.
  • Prepares detailed, accurate and timely documentation.
  • Establishes and promotes an ongoing collaborative relationship with hospital administration, physicians, nurses, discharge planners, case managers, social workers, unit clerks, homecare agencies and other facility personnel.
  • Participates in rounds meetings as required.
  • Participates in special projects and performs other duties as assigned.

The Census Management Coordinator will also have the following duties:

Leadership: The Census Management Coordinator will lead in defining and executing strategies and solutions to create business value, including building a team to design, develop, and execute those strategies and solutions to deliver desired outcomes. The Census Management Coordinator will participate in the rounds sessions and offer solutions to deliver desired census management and operational outcomes.

Strategy: The Census Management Coordinator will establish the business strategy and roadmap: (1) improve outcomes for CINQCARE Family Members; (2) enhance the efficacy of other CINQCARE. business divisions; and (3) develop and deliver external market opportunities for CINQCARE products and services. In establishing the business strategy, the Census Management Coordinator will define and innovate sustainable revenue models to drive profitability of the Company.

Collaboration: The Census Management Coordinator will ensure that capabilities form a cohesive offering, including by working closely with other business divisions to learn their needs, internalize their knowledge, and define solutions to achieve the business objectives of CINQCARE.

Knowledge: The Census Management Coordinator will provide solutions and use the subject matter expertise of the Care Team to reduce hospital lengths of stay and assist with care coordination as patients return to the community.

Culture: The Census Management Coordinator is accountable for creating a productive, collaborative, safe and inclusive work environment for the team and as part of the larger Company.

The working environment and physical requirements of the job include:

This position requires travel to local hospitals, facilities, CINQCARE practices and CINQCARE office in all types of weather conditions. In-office work is performed indoors in a traditional office setting with conditioned air, artificial light, and an open workspace.

In this position you will need an ability to travel frequently by car and/or public transportation, the ability to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 20 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
