POSITION:
Clinical Intake Coordinator

REPORTS TO:
Director of Central Intake & Census Development, VP of Network Services.

JOB SUMMARY
The Clinical Intake Coordinator plays a crucial role in the assessment and evaluation of patient referrals, ensuring timely responses to referral sources regarding acceptance or denial of patients. This position primarily focuses on hospital referrals but may also include evaluations from various healthcare facilities such as home health services, assisted living, and skilled nursing facilities.

The primary objective of this role is to enhance census growth by improving the speed of acceptance decisions and increasing the overall volume of admissions. Responsibilities encompass compiling essential patient information and communicating effectively with relevant personnel at receiving facilities. The Coordinator will maintain a daily record of reviewed patients, track denial reasons, and proactively inform staff about pending referrals to guarantee satisfaction among referral sources. Additionally, the role involves engaging in discussions regarding complex cases to address patient needs and facilitate admissions whenever feasible.

The Clinical Intake Coordinator is encouraged to identify and address any obstacles in the admission process, providing recommendations for improvement. Building and nurturing relationships with Alden Network referral partners through timely follow-ups, whether electronically, via phone, or in person, is essential.

QUALIFICATIONS
A. Preferred qualifications include an RN or LPN license.
B. A minimum of one year of experience in long-term or acute care settings is required.
C. Ability to conduct clinical reviews and determine suitable placements and care requirements for referrals.
D. Strong collaboration skills with other healthcare professionals.
E. Proficiency in Microsoft Word, Excel, and other relevant software.
F. Excellent written and verbal communication skills are essential.
G. Must be able to read, write, and speak English fluently.
H. Ability to make independent decisions when necessary.
I. Must demonstrate tact and professionalism when interacting with customers, families, and the public.
J. A personable, enthusiastic, and resourceful demeanor is essential.

ESSENTIAL FUNCTIONS
A. Timely evaluation and response to incoming hospital referrals, determining the medical complexities and appropriate placements.
B. Collaborate with facilities regarding the medical, physical, and equipment needs of accepted referrals.
C. Maintain accurate logs of all active referrals.
D. Communicate acceptance or denial reasons to referral sources.
E. Ensure accurate information is relayed to facility staff regarding bed availability and patient needs.
F. Foster positive relationships with referral providers and Alden facilities.
G. Actively participate in continuous improvement initiatives for the Alden Network.
H. Ensure 24/7 responses to referrals through coordination with intake partners and facility admissions personnel.
