Central Intake Clinical Specialist

REPORTS TO:
Director of Central Intake & Census Development, VP of Network Services.

JOB SUMMARY
The Central Intake Clinical Specialist plays a crucial role in the healthcare system by conducting clinical evaluations of patient referrals efficiently and providing timely feedback to the referring sources regarding acceptance or denial.

The primary sources of referrals include hospitals, but the Central Intake Clinical Specialist may also assess referrals from various other entities such as home health agencies, assisted living facilities, independent living centers, and skilled nursing facilities.

This position focuses on enhancing census growth by accelerating the acceptance process and increasing the volume of admissions, thereby contributing to overall admission growth.

Key responsibilities include compiling a concise summary of relevant patient information and sharing it with essential personnel at the receiving facility, maintaining a daily record of reviewed patients, tracking reasons for denials, proactively informing relevant staff if referral sources are awaiting responses to ensure satisfaction, and engaging in discussions about complex cases to meet patient needs and secure admissions.

The Central Intake Clinical Specialist is also encouraged to identify obstacles in the admission process and propose suitable recommendations.

This role involves fostering and nurturing vital relationships with Alden Network referral partners through consistent and timely follow-ups via electronic communication, phone calls, or in-person meetings.

Ideal candidates will hold either an RN or LPN license; however, non-licensed healthcare professionals with a solid background in skilled nursing and rehabilitation facilities are also considered.

This position may allow for remote work, on-site responsibilities, or a hybrid of both. Attendance at occasional in-person meetings and events to represent the Central Intake Department is required.

As this role primarily involves computer work, proficiency in technology and the ability to work independently are essential. Strong professional written communication skills are necessary, and a background in marketing or business development is advantageous.

QUALIFICATIONS
A. Preferred qualifications include a Nurse (LPN or RN) license.
B. A minimum of one year of experience in long-term and/or acute care is required.
C. Ability to perform clinical reviews and assess appropriate placement and care needs for referrals is essential.
D. Collaboration with other clinicians is necessary.
E. Proficiency in Microsoft Word, Excel, and other relevant technological programs is required.
F. Excellent written and verbal communication skills are a must.
G. Must be able to read, write, and speak English in a clear manner.
H. Ability to make independent decisions when necessary is required.
I. Must interact tactfully with customers, family members, visitors, personnel, government agencies, and the general public.
J. A personable, enthusiastic, and resourceful attitude is essential.
PHYSICAL REQUIREMENTS
A. Ability to move intermittently throughout the workday.
B. Capability to manage the mental and emotional stress associated with the position.
C. Good general health and emotional stability are necessary.
D. Ability to relate to and work with individuals who are ill, disabled, elderly, emotionally distressed, or occasionally hostile.

F. Willingness to collaborate and succeed as part of a team.
G. Perform other related duties as assigned.
ESSENTIAL FUNCTIONS
A. Timely review and response to incoming hospital referrals, determining the medical complexities and appropriate placements.
B. Collaborate with facilities regarding the medical, physical, and equipment needs of accepted referrals.
C. Track and manage all referrals actively using provided logs.
D. Communicate with referral sources regarding acceptance or denial of referrals.
E. Liaise with facility staff to ensure accurate information about bed availability, resident/patient needs, isolation, and transfers to hospitals.
F. Develop positive partnerships with referral providers, Alden Facilities, and Home Care business along with their marketing teams.
G. Actively participate in continuous improvement initiatives for the Alden Network.
H. Ensure 24/7 responses to referrals through coordination with other intake partners and facility admissions personnel.
