Essential Functions and Responsibilities
  • Manage and oversee your own patient census across various facilities to:
    • Coordinate the delivery of Chronic Care Management (CCM) services for patients with chronic illnesses in long-term care settings.
    • Develop personalized Care Plans for patients with chronic conditions.
  • Responsible for tracking and updating monthly patient information.
  • Monitor laboratory results, vital signs, medications, and testing for your patient census.
  • Facilitate care coordination with patients, their care teams, and families through various communication methods.
  • Provide education to patients regarding the management of chronic conditions, medications, and testing.
  • Maintain constant communication and collaboration with care team members such as physicians, physician assistants, nurse practitioners, pharmacists, and other healthcare professionals.
  • Identify and address clinical and non-clinical gaps in patient care.
  • Support training and development for providers and the CCS team.

Shift Structure
  • Monday-Friday, 8-hour shift (Typically 9am-5pm but can vary)
Requirements and Qualifications
  • RN in Applicable State
  • Must have active compact license
  • Reliable home internet and proficient computer skills required
  • Five years of nursing experience required
  • Strong communication and time management skills required
  • Critical thinking and problem-solving abilities required
  • Self-motivated and capable of working independently
  • Experience with chronically ill patients preferred
  • Telephonic case management or care coordination experience preferred
  • Compassionate and approachable demeanor
Physical Requirements
  • Punctuality and adherence to the company's Time and Attendance policy are essential.
  • Ability to remain seated for the majority of the shift.
