Clinical Intake Coordinator

REPORTS TO:
Director of Central Intake & Census Development, VP of Network Services.

I. JOB SUMMARY


The Clinical Intake Coordinator plays a pivotal role in the clinical evaluation of patient referrals, ensuring timely responses to referral sources regarding acceptance or denial decisions.

This position primarily engages with hospitals as referral sources, but may also involve assessments from various other entities, including home health services, assisted living facilities, independent living facilities, and skilled nursing facilities.

The emphasis of this role is on enhancing census development by streamlining the acceptance process and increasing the volume of admissions, thereby fostering growth in patient intake.

Key responsibilities include compiling concise summaries of relevant patient information and disseminating this data to essential personnel at the receiving facilities, maintaining a daily record of reviewed patients, tracking reasons for denials, and proactively communicating with staff to ensure referral sources receive timely updates, thereby enhancing referral source satisfaction. The Coordinator will also engage in discussions regarding complex cases to meet patient needs and facilitate admissions whenever feasible.

This role encourages the identification of obstacles within the admission process and the formulation of recommendations for improvement.

The Clinical Intake Coordinator will cultivate and strengthen relationships with Alden Network referral partners through consistent and timely follow-ups via electronic communication, phone calls, or in-person meetings.

Ideal candidates will possess either an RN or LPN license; however, non-licensed healthcare professionals with substantial experience in skilled nursing and rehabilitation settings may also be considered.

This position may allow for remote work, on-site presence, or a hybrid approach, with occasional in-person meetings and attendance at events to represent the Central Intake Department being required.

Proficiency in computer skills is essential, along with the ability to self-motivate and work independently. Strong professional written communication skills are mandatory, and a background in marketing or business development is advantageous.

II. QUALIFICATIONS

A. Preferred qualifications include an LPN or RN license.

B. A minimum of one year of experience in long-term or acute care settings is required.

Capability to conduct clinical reviews and assess appropriate placement and care requirements for referrals.

D. Ability to collaborate effectively with other healthcare professionals.

E. Proficiency in Microsoft Word, Excel, and other relevant technological applications.

F. Exceptional written and verbal communication skills are necessary.

C. Must be able to read, write, and speak English fluently.

D. Ability to make independent decisions when necessary.

E. Must demonstrate tactfulness in interactions with customers, family members, visitors, personnel, government representatives, and the general public.

F. A personable, enthusiastic, and resourceful demeanor is essential.

III. PHYSICAL REQUIREMENTS

A. Ability to move intermittently throughout the workday.

B. Capacity to manage the mental and emotional demands of the position.

C. Must maintain good general health and demonstrate emotional stability.

IV. ESSENTIAL FUNCTIONS

A. Promptly review and respond to incoming hospital referrals, determining the medical complexities and appropriate placements.

B. Collaborate with facilities regarding the medical, physical, and equipment needs of accepted referrals.

C. Track and manage all referrals actively using provided logs.

D. Communicate with referral sources regarding acceptance or denial of referrals.

E.

Ensure accurate communication with facility staff regarding bed availability, resident/patient needs, isolation requirements, and transfers to hospitals.


F. Foster positive partnerships with referral providers, Alden Facilities, and Home Care businesses, including their marketing teams.

G. Actively participate in continuous improvement initiatives for the Alden Network.

H. Ensure 24/7 responses to referrals through coordination with other intake partners and facility admissions personnel.
