We are seeking a full-time Client Support Specialist to join our team, focusing on our Minority Aids Initiative Grant. Role Summary:

The Client Support Specialist collaborates with an interdisciplinary team to facilitate and coordinate the continuum of care for clients, ensuring optimal use of community resources, effective service delivery, and adherence to external agency requirements. This role is essential in preparing clients for successful transitions back into their communities.

Key Responsibilities:

Reporting to the Director of Prevention and Recovery, the Client Support Specialist will:
  • Deliver case management services to clients at high risk for or living with HIV.
  • Provide education on HIV prevention, coordinate care services, and refer clients to necessary resources, including testing and treatment options.
  • Assess client needs upon program entry and collaborate with clients to set achievable goals.
  • Maintain ongoing communication with clients throughout their service journey to ensure their basic needs are met.
  • Act as a liaison with community services to support clients in achieving their identified goals.
  • Educate clients on behavioral health, promoting healthy lifestyle choices and essential daily skills.
  • Ensure client confidentiality in compliance with company policies and relevant laws.
  • Maintain accurate case files and documentation as per agency standards.
  • Complete all required documentation in a timely manner.
  • Collaborate with the MAI Coordinator and grants department for accurate data tracking and reporting.
  • Perform additional duties as assigned.
Qualifications:

Skills and Competencies:
  1. Exceptional written and verbal communication skills, with the ability to engage effectively with diverse populations.
  2. Strong understanding of cultural diversity and sensitivity.
  3. Interpersonal skills that foster teamwork and collaboration.
  4. In-depth knowledge of HIPAA regulations and confidentiality practices.
  5. Comprehensive understanding of addiction-related behaviors and emotions.
  6. Familiarity with community resources and referral systems to meet clients' diverse needs.
  7. Proficient in documenting case management activities.
Education and Experience:
  1. Bachelor's degree is preferred.
  2. A minimum of 3 years of experience in case management for at-risk populations.
  3. Experience in addiction treatment counseling is preferred.
  4. A valid driver's license and a clean driving record are required.
  5. Case Management Certification is preferred (e.g., Accredited Case Manager, Certified Case Manager, Certified Peer Support Worker).
