Overview:

Client Support Specialists deliver extensive assistance to individuals and families living in community housing.

Their role encompasses evaluation, case strategizing, and the formulation of an Independent Living Plan, alongside case coordination and management, counseling, referrals, informational workshops, and support groups. The Client Support Specialists collaborate with other program personnel and local organizations to ensure both immediate and long-term care to facilitate families' transition to stable housing.

Key Responsibilities:

The Client Support Specialist is tasked with overseeing a caseload of families and children residing in transitional housing. They provide counseling services to residents and coordinate all client-related services as outlined in the Independent Living Plan (ILP).

Primary Duties Include:

  • Conducting intake assessments for families referred to the program.
  • Managing a caseload of families referred by social services.
  • Performing home visits at least twice weekly to discuss residents' Independent Living Plans.
  • Facilitating bi-weekly progress meetings with residents to evaluate advancements.
  • Offering crisis intervention and short-term individual counseling.
  • Providing guidance and information to families regarding services and planning for permanent housing.
  • Advocating for clients in securing public assistance and entitlements from municipal agencies.
  • Managing case documentation, preparing case records, and generating monthly and quarterly statistical reports.
  • Mediating internal disputes within the living environment.
  • Leading group sessions and conducting educational presentations for community members.
  • Keeping clients informed and assisting them in addressing any challenges related to their transition to permanent housing.
  • Acting as an advocate and liaison between families and various agencies.
  • Maintaining routine administrative tasks, including correspondence preparation, file management, and communication via mail and telephone.
  • Performing additional related duties as assigned.

Required Skills and Qualifications:

Essential:

  • Strong verbal and written communication abilities.
  • Excellent interpersonal skills, with an assertive and organized approach.
  • Ability to work independently and effectively.

Educational Background:

  • A Master's degree in Social Work or a related field is preferred, with at least one year of case management experience; or a Bachelor's degree with a minimum of two years of case management experience.
  • Experience working with homeless families and understanding homelessness and residential services is preferred.
  • Strong interpersonal skills are essential.
  • Proficiency in computer applications, including Excel, PowerPoint, and Publisher.
  • Bilingual proficiency (English/Spanish) is preferred.
  • Salary range: $42,000 - $47,000 annually, commensurate with experience.
