Job Summary:

Position Overview: As a Client Care Specialist at WellLife Network, you will play a vital role in providing direct care services to individuals with developmental disabilities and acting as a liaison between our residences and medical facilities. Your primary responsibility will be to ensure that all medical appointments are carried out as required, while also collaborating with our Residence Nurse and Residence Manager for proper medical follow-up.

Key Responsibilities:

  • Collaboration and Communication: Work closely with the Residence Nurse and Residence Manager to ensure seamless medical follow-up and communication.
  • Medical Appointment Support: Accompany individuals to medical appointments and advocate for their needs, ensuring that their medical requirements are met.
  • Transportation and Safety: Operate agency vehicles according to policy, prioritizing the safety of individuals and staff.
  • Medical Documentation: Ensure that all medical documentation is obtained and accurately documented, maintaining confidentiality and adhering to regulatory requirements.
  • Training and Development: Participate in training individuals in daily living activities and implement goals to maximize their independence.
  • Medication Administration: Oversee medication administration, ensuring that it is carried out in accordance with established protocols.
  • Staff Communication: Communicate relevant information to staff, fostering a collaborative environment and ensuring that everyone is informed.
  • Crisis Intervention: Intervene in potentially dangerous situations and administer first aid if necessary, prioritizing the safety and well-being of individuals.
  • Professional Development: Complete required trainings in AMAP, SCIP, CPR, and First Aid, staying up-to-date with industry standards and best practices.
  • Confidentiality and Integrity: Maintain confidentiality and adhere to the highest standards of integrity, upholding the values of WellLife Network.

Requirements: A High School diploma or equivalent is required, with an Associate's degree in Human Services preferred. Relevant experience working with individuals with developmental disabilities and/or in the medical field is essential. A valid NYS Driver's license is also required.
