Overview


As a Clinical Care Coordinator with Catholic Health Initiatives, you will collaborate with physicians, fellow registered nurses, specialized departments, and community resources.

This role is essential in equipping patients with the knowledge and resources necessary to formulate a comprehensive care plan upon discharge.


Building trust through compassionate interactions, utilizing critical thinking skills, and recognizing that each patient requires a personalized care strategy are fundamental to your success.

You will assist patients by identifying and addressing barriers to care that may hinder their recovery. Your innovative thinking is crucial in developing enhanced discharge plans that lead to optimal outcomes for each individual.

Responsibilities

- Conduct an initial assessment of each patient's information and initiate contact.

- Engage in daily multidisciplinary rounds (MDR) and weekly length of stay (LOS) meetings to review and discuss patient care.

- Employ a collaborative care approach that evaluates clinical, spiritual, financial, and educational factors to achieve realistic outcomes.

- Design and implement discharge plans tailored to patients' needs for post-acute care services, aiming to minimize outlier days, reduce length of stay (LOS), and lower readmission rates.

- Communicate effectively with patients, their families, external vendors, post-acute care providers, and team members.

- Unencumbered license to practice in the State of Arkansas.

- Minimum of 3 years of acute care nursing experience required.

- BSN preferred, or an Associate Degree with CCM or ACM Certification within 2 years of hire date.

- Previous experience in case management or discharge planning is strongly preferred.

Pay Range

$39.52/hour
