Clinical Care Nurse

Join our dedicated clinical team at Arkansas Heart Hospital as a Clinical Care Nurse (LPN). In this role, you will support patient care initiatives by executing a range of responsibilities within the LPN scope of practice, all while adhering to the patient-centered care model and under the guidance of the Clinic Manager.
Key Responsibilities:
- Engage actively in patient care, ensuring a safe and supportive environment.
- Welcome patients and facilitate the admission process by ensuring all necessary paperwork is completed.
- Implement basic nutritional principles in patient care.
- Monitor, document, and report on patient conditions effectively.
- Coordinate patient scheduling for appointments and procedures.
- Collaborate with RNs and MDs to develop and assess patient education plans.
- Provide clear explanations of procedures and address patient inquiries regarding care.
- Assist with patient billing and charge processes as required.
- Gather essential information from healthcare providers and insurance for pre-certification.
- Identify and document changes in patient conditions across various health systems.
- Administer basic dressings and assist with more complex procedures as needed.
- Conduct thorough medication reconciliation.
- Maintain accurate and up-to-date patient records in the electronic medical record system.
- Document immunizations, including Flu and Pneumonia shots.
- Perform EKGs, obtain vital signs, and conduct phlebotomy and blood glucose monitoring.
- Travel to outreach clinics as necessary.
- Handle clerical tasks as required.
Qualifications:
Education: Completion of an accredited Practical Nurse program.
Licensure/Certification: Valid state licensure as a Practical Nurse or from a Nursing Compact state. Current BLS and ACLS certifications are required within 30 days of hire. Basic EKG certification is required within 6 months of employment, with renewal as per certification guidelines.
Experience: Preferred 6 months to 1 year of continuous experience in an acute care environment.
