Position Summary

The Community Clinical Coordinator is accountable for overseeing all facets of organizational outreach and marketing efforts. This role involves leading the marketing team, fostering and sustaining positive relationships with clients and referral sources, addressing client inquiries and concerns, and negotiating service agreements with managed care entities, governmental bodies, and other payers. The coordinator evaluates the suitability for hospice services and facilitates the transition of care from community settings to hospice services. This position also involves collaboration with the community referral network and the hospice clinical team, including the admission nursing staff.



Key Responsibilities

  • Engages in daily marketing operations, providing direct oversight for the development and execution of marketing strategies.
  • Complies with all applicable laws, regulations, and guidelines regarding ethical marketing practices, patient engagement, and accurate promotional materials.
  • Maximizes third-party reimbursement through effective negotiation of service agreements with managed care organizations, government entities, and other payers.
  • May assist in conducting market evaluations and formulating a comprehensive marketing strategy aimed at achieving budgetary targets.
  • Implements marketing and promotional strategies to meet budgetary goals.
  • Establishes and nurtures positive working relationships with existing and potential referral sources and payers.
  • Negotiates service pricing with insurance case managers and other payers within established financial parameters.
  • Enhances community, customer, payer, and patient perceptions of the hospice as a premier provider of services.
  • Leads strategic planning efforts, identifying opportunities for enhanced or additional services to meet client needs.
  • Coordinates care transitions from community services to hospice care, prioritizing the best interests of potential clients.
  • Utilizes clinical expertise to assess clients for eligibility under Medicare Hospice Guidelines across all care levels, including Routine Home Care, Respite, General Inpatient, and Continuous Care.
  • Effectively communicates assessment findings to the clinical team, including the admission nursing staff.
  • Maintains a thorough understanding of the organization's market landscape, including government agencies, major payer groups, key referral sources, and competitive positioning.
  • Stays informed about marketing trends and shares relevant information with appropriate personnel within the organization.
  • Maintains a comprehensive knowledge of community resources and assists clients in accessing these resources when hospice services are not applicable.
  • Monitors and evaluates the cost-effectiveness of marketing initiatives.

Qualifications

  • Completion of a Registered Nurse accredited program with an active state nursing license.
  • A minimum of one year of nursing experience is required, with a preference for experience in hospice care.
  • Ability to effectively market services and engage tactfully with clients, the community, and the hospice clinical team.
  • Demonstrated understanding of disease processes and comprehensive medical record evaluations.
  • Exhibits strong communication, negotiation, and public relations skills.
  • Demonstrates independence, organizational skills, assertiveness, adaptability, and collaboration in fulfilling job responsibilities.
