Position Overview
The Geriatric Care Coordinator (RN) plays a vital role in developing and assessing healthcare plans for elderly patients, those recovering from significant injuries, or individuals with chronic conditions. This position involves working in various settings, including hospitals and community environments, while collaborating with healthcare professionals to ensure comprehensive patient care.
Key Responsibilities
- Maintain confidentiality and security of patient information.
- Ensure a safe and healthy work environment by adhering to Sage Health policies and legal regulations.
- Develop and oversee care plans for patients with chronic conditions such as diabetes, heart disease, and cancer.
- Advocate for individualized treatment options that meet the unique needs of each patient.
- Coordinate post-discharge appointments for patients within 72 hours and follow up to confirm attendance.
- Communicate effectively with patients and their families regarding health conditions.
- Provide education and guidance in collaboration with primary care physicians for complex medical decisions.
- Act as a liaison between patients and insurance providers to ensure quality and cost-effective care.
- Verify daily patient census and planned discharges.
- Identify and provide resources for patients and families related to their diagnoses.
- Conduct regular meetings with clinical support staff, physicians, and operations leadership.
- Assist primary care physicians with virtual appointments in patients' homes.
- Ensure the quality of healthcare services for each patient.
- Update electronic health records with the latest medical treatments.
- Prepare for health plan audits and inspections.
- Stay informed about nursing regulations to support clinical staff.
- Perform additional duties as assigned.
Qualifications
Required:
- Bachelor's Degree in Nursing.
- Active Registered Nurse license in the state of employment.
- Basic Life Support certification.
- 2-3 years of experience in inpatient, outpatient, home health, hospice, or case management.
- Strong clinical assessment skills and experience in chronic disease management.
- Experience in culturally diverse communities.
- Excellent communication skills, both written and verbal.
- Organizational skills and attention to detail.
- Ability to manage multiple priorities in a fast-paced environment.
- Problem-solving skills and resourcefulness.
- Valid driver's license and access to a vehicle for travel between healthcare facilities.
- Experience with electronic health records and proficiency in Microsoft Office.
- Full COVID-19 vaccination is a requirement for this role.
Preferred:
- Certification in Case Management (ACM or CCM).
- Experience in managed care settings.
- Skills in motivational interviewing.
- Bilingual in Spanish and English.
