Key Responsibilities- Conduct thorough assessments of patients in Skilled Nursing Facilities (SNFs) using clinical skills and appropriate measurement tools.
- Review and update targets for Length of Stay (LOS), target outcomes, and discharge plans with healthcare providers and families.
- Complete concurrent reviews of SNF stays, updating authorizations in a timely manner.
- Collaborate with healthcare teams to establish optimal discharge plans, including physicians, referral coordinators, discharge planners, social workers, and physical therapists.
- Assure patients' progress toward discharge goals and assist in resolving barriers.
- Participate in SNF Rounds to provide accurate and up-to-date information to the naviHealth Senior Manager or Medical Director.
- Ensure appropriate referrals are made to the Health Plan, High-Risk Case Manager, and/or community-based services.
- Engage with patients, families, or caregivers through regular communication and care conferences.
- Monitor patients' continued appropriateness for the SNF setting according to InterQual criteria or the nH Outcome.
- Active, unrestricted registered clinical license as a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Therapist.
- 3+ years of clinical experience in a relevant field.
- Proficiency in Outlook, Excel, and Word.
- 2+ years of Case Management experience.
- Experience working with geriatric populations.
- Patient education background, rehabilitation, and/or home health nursing experience.
- Proven ability to be detail-oriented.
- Proven ability to be a team player.
- Demonstrated exceptional verbal and written interpersonal and communication skills.
- Proven solid problem-solving, conflict resolution, and negotiating skills.
- Proven independent problem identification/resolution and decision-making skills.
- Proven ability to prioritize, plan, and handle multiple tasks/demands simultaneously.
