Position Overview: The Transplant Care Coordinator at Houston Methodist is a registered nurse tasked with engaging in departmental initiatives and overseeing all facets of the peri-operative and post-transplant stages of transplantation.

This role requires the ability to advance in skills to manage increasingly intricate patient cases, receiving guidance from seasoned Transplant Coordinators and management, enhancing their professional practice.

Key Responsibilities:

  • Collaborates with all members of the inter-professional transplant team to ensure seamless care coordination for post-transplant patients and to achieve optimal treatment outcomes.
  • Exhibits clinical expertise and proficiency in patient care, utilizing technology effectively and managing emergencies while advocating for patient and family rights.

Essential Functions:

Communication: Engages in proactive, positive communication with transplant team members, patients, referring physicians, and other healthcare providers; accurately reports relevant patient care and family information.

Patient Advocacy: Identifies and addresses the clinical, psychosocial, and economic needs of transplant patients and their families throughout the pre-operative, hospital, and post-transplant phases.

Team Collaboration: Promotes a teamwork-oriented environment, anticipating the needs of colleagues and offering assistance as necessary.

Patient Education: Establishes educational goals with patients and their families, providing resources and information regarding follow-up care, medication adherence, and signs of complications.

Quality and Safety: Monitors patient recovery post-transplant, identifies potential complications, and collaborates with the healthcare team to implement care plans that align with current standards.

Documentation: Maintains comprehensive records of donor and recipient transplant information, ensuring compliance with regulatory standards.

Financial Management: Validates insurance authorizations for diagnostic testing and ensures proper documentation for departmental financial accountability.

Professional Development: Engages in continuous learning and participates in performance improvement initiatives to enhance departmental quality and processes.

Qualifications:

  • Bachelor's degree or higher from an accredited School of Nursing.
  • A minimum of four years of nursing experience, with a preference for previous transplant acute care or ICU experience.
  • Current RN licensure in Texas or a Compact State and BLS certification.
