As a Clinical Dietitian, you will play a vital role in evaluating the nutritional needs of our patients utilizing established assessment methodologies. You will take pride in advising and guiding patients, their families, and caregivers on tailored nutrition plans aimed at enhancing health outcomes.

Your responsibilities will include working with a diverse patient population, including those with complex medical needs, under the mentorship of senior dietitians. You will conduct comprehensive nutritional assessments, taking into account age, gender, and cultural backgrounds. Routine screenings will be part of your role to identify patients at nutritional risk, and you will collaborate within a multidisciplinary team to ensure optimal care for those requiring specialized attention, all while providing outstanding service to patients and their families.

This outpatient Clinical Dietitian position focuses on transplant patients, providing necessary nutritional support and cross-coverage as required.

Key Responsibilities
  • Formulate individualized intervention plans that specify the appropriate nutrition delivery methods and prescriptions tailored to each patient's unique requirements, following physician directives.
  • Assess and document patient responses to dietary interventions, ensuring measurable goals are met as outlined in the nutrition care plan.
  • Revise care plans as needed and maintain follow-up with patients to ensure ongoing support.
  • Work collaboratively within a multidisciplinary team to facilitate care and identify patients at nutritional risk or those needing advanced support.
  • Partner with the Food and Nutrition Services team to enhance the overall patient experience.
Qualifications
  • Bachelor's Degree with completion of an accredited dietitian program (ACEND - Accreditation Council for Education in Nutrition Dietetics).
  • Current registration as a Registered Dietitian with the Commission on Dietetic Registration (CDR).
  • Valid Dietitian State license in applicable states.
