Key Responsibilities:
This role requires a commitment to developing skills to care for patients with increasing complexity, benefiting from mentorship by seasoned Transplant Coordinators and management.
The Transplant Care Coordinator collaborates with all members of the inter-professional transplant team to ensure seamless care for post-transplant patients and to achieve optimal treatment outcomes.
Clinical Duties:
Demonstrates clinical expertise and proficiency in patient care, utilizing technology effectively and managing emergencies while advocating for patient and family rights.
Essential functions include:
- Engaging in proactive communication with transplant team members, patients, and referring physicians, ensuring comprehensive reporting of patient care and family information.
- Utilizing therapeutic communication to build rapport with patients and families, clearly articulating the care plan.
- Identifying and addressing the clinical, psychosocial, and economic needs of transplant patients and their families throughout the surgical and recovery phases.
Team Collaboration:
Fosters a collaborative environment, anticipating the needs of team members and offering assistance proactively.
Contributes to departmental goals for employee engagement and teamwork.
Patient-Centered Care:
Adheres to patient and family-centered care standards, aligning with the Houston Methodist Professional Nursing Practice framework.
Assists in discharge planning to ensure continuity of care for transplant recipients.
Education and Support:
Explains follow-up procedures and the necessity for ongoing monitoring at the transplant center.
Sets mutual educational objectives with patients and families, providing resources on vital signs, follow-up visits, lab tests, and medication management.
Quality and Compliance:
Monitors patient progress post-transplant, identifies complications, and collaborates with the transplant team to implement necessary interventions.
Maintains thorough documentation in accordance with OPTN/UNOS regulations and ensures compliance with quality standards.
Financial Responsibilities:
Validates insurance authorizations for diagnostic testing and ensures accurate documentation for departmental financial management.
Professional Development:
Engages in continuous learning and participates in performance improvement initiatives, applying evidence-based practices to enhance patient care.
