Responsibilities
As a Patient Care Coordinator RN, you will play a vital role in managing patient care throughout their healthcare journey. Your responsibilities will include:
- Coordinating care and services for patients across various stages of illness.
- Promoting efficient utilization of healthcare resources.
- Leading interdisciplinary teams to achieve optimal patient outcomes through effective case management.
- Conducting comprehensive patient assessments and developing tailored discharge plans.
- Ensuring timely discharges by evaluating post-discharge needs and arranging necessary services.

Qualifications
Experience/Education:
Minimum of two (2) years of clinical experience in an acute care setting, with either an Associate Degree in Nursing or a Master's degree in Case Management, or a Master's Degree in Nursing with a valid TX RN License.
Licensure/Certifications:
Current Registered Nurse (RN) license.
