Position Overview
The RN Clinical Communications Coordinator at Houston Methodist is a vital role for registered nurses, tasked with the facilitation and orchestration of patient care throughout the transplantation journey. This position emphasizes professional nursing care for a diverse patient demographic and involves collaboration with all members of the interprofessional transplant team to guarantee seamless care for transplant patients. Responsibilities include indirect patient care that addresses varied and intricate needs.

Key Responsibilities
  • Patient Advocacy: Act as the primary contact for the department, utilizing effective telephonic communication throughout all transplant phases. Employ therapeutic communication techniques, including active listening and education, to build rapport with patients and their families. Recognize and address the clinical, psychosocial, and economic needs of transplant patients and their caregivers.
  • Interprofessional Communication: Maintain ongoing communication with candidates, referring physicians, healthcare providers, payors, and transplant team members regarding patient status. Provide telephonic education on transplant and organ donation.
  • Team Collaboration: Foster a collaborative environment by anticipating the needs of team members and proactively offering support. Serve as a resource for referring physicians and contribute to enhancing departmental employee satisfaction.
Service Excellence
  • Adhere to patient and family-centered care standards as outlined in the Houston Methodist Professional Nursing Practice document. Coordinate with the healthcare team for the evaluation of potential transplant candidates, ensuring alignment with cultural diversity and inclusion principles.
  • Advocate for patients and families to address clinical and ethical concerns, utilizing appropriate resources. Assess educational needs throughout the transplant process and provide additional education as necessary.
  • Ensure that both patients and referring physicians receive written communication regarding the transplant referral and evaluation processes.
Quality and Safety
  • Identify suitable candidates upon referral and engage with relevant staff. Evaluate candidate requirements, including co-morbidities and significant changes, and communicate findings with the transplant team.
  • Provide updates to patients regarding their candidate listing status in accordance with OPTN/UNOS regulations.
  • Maintain comprehensive documentation of all communications and ensure timely resolution of inquiries.
Financial Responsibilities
  • Verify patient insurance authorization for transplant referrals and coordinate with the financial team to secure necessary approvals.
  • Demonstrate self-motivation in managing time effectively and prioritizing tasks to meet departmental financial targets.
Professional Development
  • Engage in departmental initiatives aimed at quality improvement and performance enhancement. Collaborate with team members to develop solutions and participate in shared governance activities.
  • Identify personal learning needs and pursue educational opportunities to enhance professional skills and knowledge.

Qualifications
  • Bachelor's degree or higher from an accredited School of Nursing.
  • Minimum of one year of nursing experience in a healthcare setting.
Licenses and Certifications
  • RN - Registered Nurse - Texas State Licensure and/or Compact State Licensure within 60 days.
  • BLS - Basic Life Support (AHA) certification.

  • Proficient in English, with strong communication skills for patient and team interactions.
