POSITION SUMMARY

The Community Outreach Specialist is tasked with promoting and facilitating program objectives and engaging with the community and patients in relevant sectors.

KEY RESPONSIBILITIES

The Outreach Specialist will perform a variety of essential functions, including but not limited to:

  • Conduct outreach initiatives targeting specific populations through various methods such as phone calls, mail, community visits, and other approved engagement strategies;
  • Build and nurture relationships with clients;
  • Deliver thorough education and information via street outreach, health fairs, educational institutions, community events, and other sanctioned activities;
  • Participate in scheduled and spontaneous meetings while maintaining open communication with team members and supervisors;
  • Assist in generating required reports for data collection on a weekly, monthly, and/or quarterly basis;
  • Keep outreach activity logs and calendars in accordance with program standards;
  • Support program evaluations and updates for assigned services;
  • Attend relevant community resource meetings and training sessions as assigned;
  • Collaborate with other departments as necessary;
  • Create, modify, and distribute outreach materials;
  • Facilitate classes, training sessions, and community meetings as needed;
  • Perform additional duties as assigned or modified based on organizational needs.

QUALIFICATIONS

Education: (Preferred)

  • A high school diploma or GED is required; a Bachelor’s degree in a related field is preferred;

Experience: (Preferred)

  • Two years of relevant experience in human services and/or public health;
  • Two years of experience in community and/or street outreach, particularly with hard-to-reach populations;
  • Experience in community organizing;
  • Experience working with underserved communities, including undocumented workers, unaccompanied minors, LGBT individuals, communities of color, youth/adolescents, and those facing homelessness, substance abuse, and/or mental health challenges;
  • Bilingual proficiency in English and Spanish is required (reading, writing, speaking).
