Overview

Under the guidance of the Program Manager and Program Supervisor, the Community Support Specialist will deliver professional case management services aimed at outreach and connection for individuals experiencing homelessness. This role involves assisting clients and their support networks in identifying and developing strategies to address the challenges associated with homelessness.

The Specialist will receive ongoing technical support and supervision to enhance their skills in case management practices tailored to assist homeless individuals.

Utilizing evidence-based methods, the Specialist will assess clients' immediate needs, housing situations, and collaborate on creating effective treatment plans to address issues contributing to their homelessness.

Daily responsibilities will require coordination of fieldwork to serve clients, complete necessary documentation, attend meetings, and participate in training sessions.

Collaboration with a multidisciplinary team in the field is essential as part of the outreach efforts in the designated service area.


Key Responsibilities

- Conduct thorough assessments of clients' immediate needs to develop comprehensive treatment plans.

- Collaborate effectively within a Multi-Disciplinary Team (MDT) to ensure holistic support for clients.

- Provide assistance to homeless individuals in various settings, ensuring a compassionate approach.

- Support clients in formulating treatment plans that align with the evaluations of healthcare professionals.

- Facilitate client orientation to available services and resources, including recovery support and self-help opportunities.

- Assist clients in identifying educational and employment training opportunities to promote self-sufficiency.

- Develop personalized post-service care plans to aid clients in transitioning to stable living conditions.

- Maintain regular follow-up with clients to ensure ongoing stability and support.

- Document all interactions and treatment plans in accordance with established guidelines.

- Provide essential case management services, including addressing immediate needs such as food and hygiene support.

- Engage with clients facing co-occurring issues, ensuring effective documentation and compliance with care standards.

- Participate in all required meetings, training sessions, and collaborative efforts to enhance service delivery.

- Maintain accurate records of client services while adhering to confidentiality and ethical standards.


Qualifications

- High school diploma or equivalent; certification as a Registered Alcohol and Drug Abuse Counselor is preferred.

- Lived experience with homelessness and a background in outreach or peer counseling is essential.

- Ability to engage with clients from diverse backgrounds and communicate effectively.

- Proficiency in Microsoft Office applications and familiarity with data management systems.

- Must be able to navigate challenging environments and work effectively under pressure.

- A minimum of three years of sobriety is required for candidates in recovery.

- Valid California ID and completion of a TB test are mandatory prior to employment.


Working Conditions

This position involves working in various environments, including outreach in homeless encampments and areas with high crime rates. The Specialist must be prepared for potential exposure to challenging situations and must maintain a professional demeanor at all times.

Physical demands include sitting, standing, walking, and carrying light loads as necessary. Mental resilience is required to manage unpredictable situations and client interactions.
