The Nurse Care Manager is responsible for leading and delivering individualized, rehabilitative, and comprehensive care while managing chronic health conditions for our residents. Our Health Center embraces the Small House Model of care, promoting a resident-centered approach that empowers individuals in their daily lives, care decisions, and living environments.

This role involves three 12-hour shifts, including every other weekend, with competitive compensation up to $32.75/hour for LPNs and $38.00/hour for RNs.

Available Positions:

Full Time - 3/12-hour shifts: 7:00pm - 7:30am (Nights) or 7:00am - 7:30pm (Days)

Part Time - every other weekend: 7:00pm - 7:30am (Nights)

PRN - as needed: 7:00pm - 7:30am (Nights) & 7:00am - 7:30pm (Days)

Position Overview

The Nurse Care Manager will oversee and coordinate the healthcare needs of residents, ensuring they receive exceptional care and support.

Key Responsibilities:
  • Utilize expertise in various medical fields to assess resident needs and develop tailored care plans.
  • Coordinate care activities, including scheduling, consultations, and monitoring treatment plans.
  • Collaborate with healthcare professionals to ensure seamless service delivery and optimal outcomes.
  • Conduct utilization management to enhance resource efficiency and patient care.
  • Provide education to residents on health management and preventive strategies.
  • Maintain accurate documentation and patient records.


Qualifications:
  • Must be a licensed nurse (RN or LPN) in North Carolina with 3-5 years of experience; geriatric nursing preferred.
  • At least two years of experience as a Charge Nurse or Nurse Supervisor, including direct supervisory experience.
  • Commitment to promoting person-centered care standards.
  • Strong computer skills; experience with Electronic Health Records preferred.
  • Ability to read, write, and understand English.
  • Certification in ServSafe and BLS/CPR required within the first 90 days of employment.
  • Knowledge of nursing and medical assessment practices and relevant regulations.
  • Able to plan, supervise, coordinate, and evaluate nursing team activities.
