The Case Manager will provide essential hands-on case management services to clients in the community. This role involves helping clients establish connections to long-term community support, focusing on five key areas for community stability: psychiatric treatment and medication adherence, financial management, substance abuse support, housing crisis intervention, and family engagement. The primary objectives are to maintain client engagement and support their stability in a residential environment throughout the duration of services.
Position Summary
The Case Manager will assess the resources necessary to address the unique needs of each participant. Responsibilities include advocating for participants to ensure they receive required services, identifying program needs, recognizing service gaps, and recommending effective strategies. A commitment to RHD's core values is essential in executing these duties.
Key Responsibilities
Team Collaboration
- Participate as a member of a multidisciplinary team to deliver services from a holistic and values-driven perspective.
- Contribute to the development and review of recovery plans in partnership with participants, clinical staff, and other stakeholders.
- Establish and maintain relationships with community organizations to assist participants in accessing available resources.
Direct Client Engagement
- Deliver community-based services to participants, focusing on education, resource connection, community involvement, and achieving recovery plan objectives.
Administrative Duties
- Complete all necessary assessments and documentation as mandated by RHD, funding sources, and licensing/accreditation bodies.
- Perform additional duties as assigned.
Qualifications
- Bachelor's Degree in Human Services or a related field.
- Intermediate proficiency in computer skills.
- Preferred: One year of Case Management experience.
- Required: Two years of general work experience supporting individuals in behavioral health, addiction recovery, or IDD services.
- Valid driver's license.
Working Conditions
Additional Special Working Conditions:
- Community-based work within the service delivery area. Driving program participants to appointments is required.
Physical Requirements
- Medium lifting: exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.
Physical Abilities
- Ability to stand or sit for extended periods.
- Walking and using hands or fingers for various tasks.
- Climbing stairs or ladders.
- Effective communication skills (talking and hearing).
- Visual acuity for detecting and assessing information.
- Ability to push, pull, and perform repetitive motions.
- Reaching, kneeling, stooping, crouching, or crawling as needed.
