Ideal Candidate Profile:
  • Bachelor's Degree in a Human Services field.
  • Valid Driver's License.
  • Ability to reliably commute throughout the designated service area.
  • Successful completion of all required background checks and clearances.
  • Willingness to work in various community settings and travel with clients for appointments.

Program Overview:
Our program focuses on providing trauma-informed, comprehensive case management services to adults facing homelessness who also have significant mental health challenges. The primary goal is to connect individuals with suitable, affordable housing and offer support during their transition from temporary accommodations to stable living situations. We facilitate access to mental health and substance use services, collaborate with community providers, and help clients build their support networks. Our approach includes ongoing monitoring and assessment to ensure effective support during this critical period.

Position Responsibilities:
  • Engage in case management meetings at various community locations.
  • Participate in individual and team supervision sessions as directed.
  • Accompany clients to appointments and community meetings.
  • Advocate for clients to help them access necessary resources.
  • Maintain regular contact with clients and service providers to monitor service delivery.
  • Establish connections to both formal and informal community supports.
  • Regularly assess clients' progress and adjust support as needed.
  • Document all client interactions and maintain accurate records.
  • Complete all required documentation for clients as mandated by the program.
  • Meet weekly productivity requirements as established by management.

Qualifications:
  • A Bachelor's degree in social work, psychology, counseling, or a related field.
  • Valid Driver's License required.
  • Experience working with homeless populations and providing outreach is preferred.
  • Familiarity with mental health practices is highly desirable.
  • Knowledge of Recovery Model and Trauma-Informed care.
  • Experience with Motivational Interviewing and Stages of Change methodologies.
  • Proficiency in Microsoft Office Suite.
  • Ability to respond to clients in crisis within a timely manner.
  • Professional interaction with clients and service providers using a trauma-informed approach is essential.
  • Team-oriented with a flexible approach to work.
  • Willingness to work in various community settings.
