The Case Manager is tasked with delivering direct case management services to clients in the community. This role involves aiding clients in establishing connections to sustainable community resources, focusing on five key areas that promote stability: mental health treatment and medication adherence, financial management, substance use support, housing crisis intervention, and family support. The primary objectives are to maintain engagement with clients and ensure their stability in a residential environment throughout the duration of services provided.

Position Summary

The Case Manager will assess the resources necessary to address the unique needs of each participant. This role includes advocating for participants to secure essential services, identifying program needs, and suggesting strategies to fill service gaps. A strong commitment to RHD's values is essential in executing job responsibilities.

Key Responsibilities

Collaborative Planning

  • Participate as a member of a multidisciplinary team to deliver services from a holistic and values-driven perspective.
  • Assist in formulating recovery plans and conducting reviews in partnership with program participants, clinical staff, and other relevant stakeholders.
  • Establish and nurture relationships with community organizations to aid program participants in accessing available resources.

Direct Client Engagement

  • Deliver community-based services to program participants, emphasizing education, resource connection, community involvement, and achievement of recovery goals.

Administrative Duties

  • Complete all required assessments and documentation as mandated by RHD, funding sources, and licensing/accreditation bodies.
  • Perform other duties as assigned.

Qualifications

  • Bachelor's Degree in Human Services or a related field.
  • Intermediate proficiency in computer skills.
  • Preferred: One year of experience in Case Management.
  • Required: Two years of general work experience supporting individuals in behavioral health, addiction recovery, or intellectual/developmental disabilities services.
  • Valid driver's license.

Working Conditions

Special Working Conditions:

  • Community-based work within the service delivery area of the program, including transporting participants to appointments.

Physical Requirements

  • Medium lifting: exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.

Physical Abilities

  • Ability to stand or sit for extended periods.
  • Ability to walk and perform manual tasks.
  • Ability to communicate effectively.
  • Ability to see and assess various conditions.
  • Ability to engage in repetitive motions and reach in various directions.
