Position Title: Housing Navigation Coordinator

Department: Houseless Intervention Services

Reports to: Program Manager

Compensation Level: Entry Level $28.05

Classification: Non-exempt

Position Summary: The Housing Navigation Coordinator will be an integral part of a committed team focused on assisting individuals facing homelessness within shelter and emergency housing environments. This role involves collaborating with Case Managers and managing a personal caseload to facilitate participants' housing searches and placements while utilizing supportive services to help maintain stable housing. The Housing Navigation Coordinator will also support the Program Manager in coordinating program activities, including the assessment and management of documentation for the Rapid Rehousing initiative.

Key Responsibilities:

  • Navigate the intricate landscape of social and housing services within the Metro area.
  • Collaborate with the Program Manager to ensure accurate daily management of participant rosters, including intake, exit, and follow-up documentation.
  • Maintain organized physical and electronic records, utilizing the Homeless Management Information System (HMIS) for participant data management and reporting.
  • Engage in ongoing community partner meetings to enhance service delivery.
  • Work alongside community partners to identify and address participant needs effectively.
  • Ensure clear communication with all team members and relevant stakeholders.
  • Perform additional tasks and responsibilities as assigned.
  • Comply with all applicable state and federal privacy regulations and organizational policies regarding confidentiality and security.
  • Conduct home visits or attend group meetings to provide information about agency services and procedures.
  • Prepare and review reports with the supervisor.
  • Submit requests for assistance and maintain HMIS data integrity.
  • Assist participants with rental assistance applications and conduct navigation assessments to identify barriers to access.
  • Meet regularly with participants to monitor progress and provide support.
  • Document daily interactions and maintain cleanliness of the facility.
  • Foster connections with community partners and stakeholders.
  • Track housing opportunities and conduct resource mining.
  • Maintain communication with staff and participants regarding housing stability plans.
  • Educate participants on healthy tenancy practices.
  • Participate in team meetings and training sessions as required.
  • Provide information to supervisors and colleagues in various formats.
  • Document follow-up calls for participants who have exited the program.

Supervisory Duties:

  • None

Required Experience:

  • 1-2 years of experience in a similar role within social services.
  • Familiarity with client information databases and operational strategies.
  • Experience in administrative or support services.
  • Understanding of human behavior and performance.
  • Knowledge of customer service principles.

Required Skills:

  • Understanding of Housing First and Harm Reduction principles.
  • Adaptability to change and variety in the workplace.
  • Ability to engage effectively with diverse populations.
  • Strong communication skills in various formats.

Preferred Education:

  • Bachelor's Degree in Social Work, Psychology, or a related field.
  • Relevant certifications or training.
  • Lived experience in the field.

Additional Requirements:

  • Physical ability to perform assigned tasks.
  • Successful completion of a background check.
  • Ability to work in a drug-free environment.

Work Environment:

Work may involve both indoor and outdoor settings, with exposure to varying temperatures and conditions.

Physical Demands:

Ability to lift up to 50 lbs, with tasks requiring bending, stooping, and standing for extended periods.

Travel Requirements:

Occasional travel to other work sites may be necessary.
