Current Opportunity: We are seeking a dedicated Housing Coordinator to join our esteemed Recuperative Care Program (RCP) Team.

Work Schedule: Monday - Friday (8:00 AM - 4:30 PM)

MINIMUM QUALIFICATIONS:

  1. Essential: A combination of education and experience that equips you to fulfill the responsibilities of this role, including a minimum of two years of experience in a pre-hospital or post-hospital environment. Experience working with individuals facing mental health and addiction challenges is highly desirable. Relevant experience may include:
    1. Housing Placement and Retention
    2. Social Work or Behavioral Health
    3. Case Management or Care Coordination
    4. Substance Use Disorder Treatment
  2. Essential: At least two years of experience in low-income housing or a comparable setting.
POSITION SUMMARY: The Evergreen Crossing initiative is a pioneering program designed to provide integrated housing, health care, and employment services, with the ultimate aim of eliminating homelessness. We strive to transform the delivery of these services for all individuals, particularly those in vulnerable situations. This role involves collaborating with a multi-disciplinary team at Evergreen Crossing to deliver comprehensive, person-centered, integrated services to the Eastside community of Portland.

The Housing Coordinator will work closely with participants in the Recuperative Care Program to assist them in identifying, securing, and transitioning into affordable permanent housing, transitional options, or other placements upon discharge from the program. The RCP collaborates with partner hospitals and referral agencies to offer recuperative care that addresses both acute and ongoing medical needs, while also providing short-term emergency supportive housing, crisis management, and referrals to appropriate treatment services. The Housing Coordinator will coordinate with case management staff and community partners to develop and implement housing and treatment plans.

KEY RESPONSIBILITIES:
  1. Collaborate as part of a multi-disciplinary team to provide comprehensive, client-centered, and trauma-informed services.
  2. Engage in the integration of services at Evergreen Crossing and identify opportunities to enhance collaboration among service and care teams while adhering to information-sharing protocols.
  3. Identify and pursue opportunities for workflow and logistical improvements within Evergreen Crossing. Participate in the review of Key Performance Indicators and contribute to continuous improvement initiatives.
  4. Foster an understanding of and appreciation for diversity among clients, facilitating their access to culturally responsive and specific services as needed.
  5. Explore affordable housing options for RCP participants both within and outside the Central City Concern system.
  6. Collaborate with other Recuperative Care and Evergreen Crossing staff to ensure effective utilization of primary and specialty care services.
  7. Assist program participants in completing necessary documentation for housing acquisition.
  8. Participate in housing and placement-related meetings to effectively engage with landlords and building staff.
  9. Support program participants in building relationships with landlords and resident services.
  10. Maintain accurate records of housing applications, lease agreements, and other relevant documents for RCP participants.
  11. Provide ongoing support to program participants placed in the community as needed after their transition from RCP.
  12. Advocate for program participants as necessary, attending relevant meetings such as landlord/tenant discussions.
  13. Acquire knowledge of landlord-tenant laws, Shelter-Plus Care voucher systems, Section Eight, and other relevant programs.
  14. Assist participants with moving, securing furnishings, and making necessary improvements to their living spaces to promote housing stability.
  15. Cross-train with colleagues to provide coverage during breaks and absences.
  16. Complete all required documentation and electronic records accurately and promptly.
  17. Attend all mandatory training sessions provided by CCC.
  18. Adhere to all state and federal privacy regulations, including HIPAA, and CCC policies regarding confidentiality and security.
  19. Participate in staff meetings and training sessions as scheduled.
  20. Engage in professional development opportunities to enhance skills and knowledge in addiction and mental health.
  21. Establish relationships with community resources as appropriate.
  22. Perform additional duties as assigned.
