Client Services Coordinator

Position Overview:
The Case Manager plays a pivotal role in the management of client services, ensuring that all documentation and client-related activities are conducted efficiently. Under the guidance of the Facility Director, the Case Manager is tasked with identifying and addressing the unique needs of each client from the point of admission through to their successful transition out of the program.
Key Responsibilities:
1. Assess and document client progress and challenges throughout their stay.
2. Facilitate communication regarding clients' adaptation to the program.
3. Prepare clients for reintegration into the community, ensuring they have the necessary resources and support.
4. Maintain a flexible schedule to accommodate client access, primarily during evening hours.
5. Provide 24-hour on-call crisis intervention services as needed.
Qualifications:
Essential Knowledge:
- Familiarity with community resources and services.
- Understanding of the population served.
- Proficiency in record-keeping practices and procedures.
- Competence in computer applications and spreadsheet management.
Required Skills:
- Effective oral and written communication abilities.
- Strong organizational and planning skills for program activities.
Educational Background:
- A Bachelor’s Degree in Social Work or Counseling from an accredited institution is required.
- Submission of a writing sample demonstrating clinical documentation skills is mandatory for employment.
Licenses and Certifications:
- A valid Driver's License and proof of insurance are necessary.
- Verification of educational credentials will be required for employment.
Compensation:
