Position Overview:
General Description:
Role Summary: The Case Manager, under the guidance and oversight of the Facility Director, is
accountable for all client-related services and documentation. The Case Manager is tasked with
identifying and addressing the unique needs of assigned clients, starting from admission and
culminating with discharge. This role involves communicating the client's adaptation to the program and preparing the client
for a successful reintegration into the community. The Case Manager is expected to work a 40-hour week during
hours that facilitate client access (primarily evening hours) and provide 24-hour on-call crisis support
as Staff Duty Officer (SDO) when necessary.

• Planning and organizing program activities
Education Requirements:
• Baccalaureate Degree in Social Work or Counseling from an accredited institution
• An approved writing sample demonstrating clinical documentation will be required for
employment.
Licenses and Certification:
• Valid Driver's License and proof of insurance
• Proof of education and credentials will be required for employment
