Job Summary

We are seeking a highly skilled and compassionate Case Manager/Service Coordinator to join our team at Darke County Recovery Services. As a key member of our staff, you will play a vital role in providing exceptional support to our clients and their families.

Key Responsibilities
  • Client Assessment and Support
    • Conduct thorough assessments of client needs and develop personalized treatment plans
    • Provide ongoing support and guidance to clients and their families
  • Service Coordination
    • Coordinate interventions with a multidisciplinary team to ensure comprehensive care
    • Develop and maintain relationships with community resources and service providers
  • Crisis Intervention and Support
    • Provide crisis intervention and support to clients and their families
    • Develop and implement crisis plans to ensure client safety and well-being
  • Documentation and Reporting
    • Maintain accurate and timely documentation of client interactions and progress
    • Prepare and submit reports to management and other stakeholders as required
Requirements
  • Education and Experience
    • A High School Diploma/GED is required with 3 or more years of experience working with the SPMI/PMI population
    • An Associate's degree in a Social Services or Psychology related field, with less than 3 years experience is preferred
  • Skills and Abilities
    • Excellent communication and interpersonal skills
    • Ability to work effectively in a fast-paced environment
    • Strong problem-solving and crisis management skills
    • Proficiency in computer software and technology

Working Conditions

The Case Manager/Service Coordinator will work in a variety of settings, including private residences, community centers, and other locations as needed. The employee must be able to tolerate noxious smells, pets, and other environmental factors that may be present in these settings.

The employee must have a valid driver's license and a safe driving record, and be willing to transport clients to various locations within and outside of the county.
