Community Outreach Specialist

Under supervision, the Community Health Worker will engage with individuals and families in various settings, including homes and community facilities, to deliver essential outreach services and support social interventions related to health management.
Key Responsibilities:- Identify and recruit potential clients for various health programs, ensuring they receive appropriate services.
- Provide comprehensive information regarding program goals, eligibility, and benefits while maintaining confidentiality.
- Coordinate appointments with healthcare providers, ensuring clients are reminded and supported in attending.
- Assist clients in accessing necessary transportation and childcare services for their health needs.
- Follow up with clients to maintain engagement and support, including locating those who may have lost contact.
- Act as a liaison between clients and community resources, facilitating access to necessary services.
- Conduct assessments of clients' living conditions to identify any health-related risks.
- Support the health center's operations by ensuring clients are properly registered and welcomed.
- Organize and implement community health events, such as workshops and health fairs.
- Document outreach activities and report any issues to the program administrator.
- Maintain accurate records of outreach efforts and client interactions.
- Achieve enrollment targets as set by program funding requirements.
- Foster positive relationships within the community to enhance program visibility and support.
- Continuously update knowledge of community resources to assist clients effectively.
- Implement care interventions as directed by the healthcare team.
Education:
A High School Diploma or equivalent (GED) is required.
Experience:
Two years of experience in outreach services, including report writing and record maintenance.
- Strong oral and written communication skills.
- Ability to build and maintain effective working relationships.
- Discretion in handling sensitive information.
- Strong multitasking abilities with attention to detail.
- Critical thinking and interpersonal skills to identify and address barriers.
- Valid driver's license may be required for job-related travel.
- Knowledge of community health issues and resources.
Ensures safety in all job functions and participates in safety initiatives. This includes:
- Addressing any observed risks to patient safety.
- Reporting adverse events to management.
- Identifying potential risks in processes and communicating them appropriately.
