In this role, you will assist individuals in navigating available services, directing them to suitable resources, and fostering awareness about health prevention, particularly concerning hypertension and social isolation. Working alongside the West Baltimore RICH Collaborative team, your contributions will be essential in outreach and health screening efforts.

Key Responsibilities:

  • Build supportive relationships with individuals and their families to promote active participation in their health management.
  • Facilitate appointment scheduling and conduct home visits as required.
  • Collaborate with the healthcare team to ensure clients comprehend care plans and self-management objectives, addressing cultural and socio-economic challenges.
  • Promptly communicate patient/client concerns to ensure seamless coordination of care with the broader team.
  • Conduct eligibility assessments, enrollment, and follow-up with uninsured individuals.
  • Engage in outreach and community health screenings to raise awareness of services offered by the West Baltimore RICH Collaborative.
  • Participate in community events that may take place outside of standard hours.
  • Provide referrals for community resources as necessary.
  • Maintain accurate and timely documentation in line with organizational standards.
  • Assist individuals in arranging transportation for appointments.
  • Note: Transporting patients is not permitted.
  • Perform additional assigned responsibilities as needed.
