Position Summary:

Under supervision, the Community Health Worker is responsible for engaging with individuals and families in various settings, including homes and community facilities, to deliver essential outreach services and support social interventions related to new health diagnoses.

This role involves direct interaction with patients, potentially at their bedside, to facilitate enrollment in outreach programs. Once enrolled, communication with patients may occur through various channels, including phone calls, in-person meetings, or written correspondence. The Community Health Worker collaborates closely with healthcare providers and community organizations to enhance patient outcomes.

Key Responsibilities:
  1. Identify and recruit potential clients for various care programs, including Transitional Care Coordination and Mobile Integrated Health.
  2. Provide comprehensive information to clients and families regarding program goals, eligibility, and available services.
  3. Coordinate appointments with healthcare providers, ensuring clients are reminded and supported in attending.
  4. Assist clients in securing transportation and childcare as needed for medical appointments.
  5. Follow up with clients to maintain engagement and address any barriers to participation.
  6. Act as a liaison between clients and community resources, including social services.
  7. Conduct assessments of clients' living conditions to identify any health and safety concerns.
  8. Facilitate the health center process by assisting clients with registration and appointment logistics.
  9. Organize and support community health events, such as workshops and health fairs.
  10. Document outreach activities and maintain accurate records of client interactions.
  11. Achieve established enrollment targets and maintain positive relationships within the community.
  12. Continuously update knowledge of community resources to assist clients effectively.
Qualifications:

Education:
A High School Diploma or equivalent (GED) is required.

Experience:
Minimum of two years of experience in outreach services, including report writing and record maintenance.

Skills:
Strong communication skills, ability to build effective relationships, and capacity to manage sensitive information with discretion.

Demonstrated ability to multitask while maintaining attention to detail and critical thinking skills.

Possession of a valid driver's license may be required for travel related to job duties.

Commitment to Patient Safety:
