Job Title: Community Outreach Coordinator
The University of Maryland School of Medicine is seeking a dedicated Community Outreach Coordinator to join their team. As a Community Outreach Coordinator, you will play a vital role in assisting patients in accessing services, referring them to the appropriate resources, and increasing awareness around health prevention related to hypertension and social isolation.
Key Responsibilities:
- Establish and Maintain Relationships: Develop supportive relationships with patients and their families to encourage active engagement in their healthcare.
- Appointment Scheduling and Home Visits: Assist with appointment scheduling and conduct home visits when necessary to ensure patients receive the care they need.
- Care Team Collaboration: Work closely with the care team to ensure patients understand care plans and self-care management goals, breaking down cultural and socio-economic barriers.
- Issue Communication: Communicate patient issues promptly to ensure coordination of care with the rest of the team.
- Eligibility Determination and Enrollment: Conduct eligibility determination, enrollment, and follow-up with uninsured patients to ensure they receive necessary services.
- Outreach and Community Health Screenings: Participate in outreach and community health screenings to promote services within the community.
- Community-Based Events: Engage in community-based events that may occur outside regular hours to promote health awareness and services.
- Referrals and Documentation: Provide referrals for community services as needed and ensure accurate and timely documentation in accordance with organizational policies.
- Transportation Assistance: Assist patients in arranging transportation to appointments, while adhering to organizational policies and procedures.
