Job Summary

RCA Healthcare Management LLC is seeking a highly skilled Admissions Screener to join our team. As an Admissions Screener, you will be responsible for coordinating all admissions and transfers to our facility, working closely with nursing staff and social services to ensure the best possible care for our residents.

Key Responsibilities
  • Screen and recommend patients for long-term or subacute care
  • Visit designated hospital assignments daily to assess patient needs
  • Coordinate room changes and inter-facility transfers upon request and following approval of resident's physician
  • Interact with residents and family members, co-workers, and clinical and ancillary staff in a non-judgmental, supportive, and calm manner
  • Maintain a daily census record for the assigned facility
  • Perform additional tasks as assigned by management
Requirements
  • Bachelor's Degree or equivalent experience in a related field
  • Experience in Admissions Screening and long-term care is preferred
  • Must be flexible, professional, and reliable
  • Strong customer service skills
  • Must possess a valid New York State Driver's License and have transportation available for job-related travel
