Responsibilities- Coordinate prospective admissions with the Director of Nursing and Administration.
- Obtain and complete all necessary admission and readmission paperwork.
- Ensure that all follow-up consults are created for the Unit Secretary to schedule in a timely fashion.
- Complete all nursing assessments, including initiating the Molst.
- Ensure Facility compliance with all state requirements and facility policies, procedures, and standards of conduct.
- Assess, plan, implement, and evaluate nursing care according to the individualized needs of patients.
- Communicate and collaborate with the interdisciplinary team and physicians.
- Complete all paperwork for the Nursing Admission Assessment.
- Enter all data into Electronic Medical Record.
- Obtain orders from MD and ensure accuracy of Admission Orders in EMR.
- Complete the Admission Nursing Note.
- Complete the Fall Risk Assessment and interventions care plans as indicated (sensors, alarms, etc.).
- Complete the Pressure Ulcer Risk Assessment/Braden Scale. If high risk, ensure all preventative measures are implemented.
- Current RN license.
- Long-term care experience preferred, with admissions experience.
- Reliable transportation and demonstrate excellent work ethic.
- Excellent interpersonal, communication, and organizational skills.
