Position Overview:

The Advanced Practice Provider (APP) in Palliative Care plays a crucial role in delivering specialized medical services to residents in long-term care facilities. This position emphasizes a collaborative approach with the primary care team to ensure comprehensive patient care.


Key Responsibilities:
  • Provide primary and urgent care services to nursing center residents.
  • Work closely with the Center Leadership Team to meet specific healthcare goals.
  • Deliver palliative care services focusing on pain management, psychosocial support, and advance care planning.
  • Act as a consultant within the interprofessional team, offering recommendations that align with patient preferences.
  • Educate patients, families, and healthcare teams on treatment options and shared decision-making processes.

Qualifications:

The ideal candidate will possess the following qualifications:

  • Graduation from an accredited School of Nursing program.
  • Licensure as a registered nurse and certification as a Nurse Practitioner with a Master's Degree in Nursing.
  • Specialization in Older Adult or Family Nurse Practitioner (FNP) is preferred, with consideration for other specialties based on experience.
  • A minimum of two years of experience working with adult or geriatric populations is preferred.
  • Possession of an unrestricted professional license and DEA in states where practice allows.
  • Palliative care certification is preferred.
