We are seeking a skilled Clinical Liaison to facilitate referrals and coordinate care for patients in our Hospice and Palliative Care programs.
Key Responsibilities- Facilitate referrals from all sources and obtain reviews to complete the clinical assessment of patients for the correct program and location.
- Assume dual accountability for Palliative Care and Hospice referrals.
- Coordinate with healthcare teams to ensure seamless care transitions.
- Develop and maintain relationships with healthcare providers and community partners.
- Associate's Degree or Diploma in Nursing
- New Mexico RN License
- Minimum one year of acute care experience, with hospice experience preferred
- Basic typing competency
- Current driver's license and auto insurance
- BLS certification
- Hospice/Palliative Care experience
