About the Role

We are seeking a skilled Clinical Liaison to facilitate referrals and coordinate care for patients in our Hospice and Palliative Care programs.

Key Responsibilities
  • Facilitate referrals from all sources and obtain reviews to complete the clinical assessment of patients for the correct program and location.
  • Assume dual accountability for Palliative Care and Hospice referrals.
  • Coordinate with healthcare teams to ensure seamless care transitions.
  • Develop and maintain relationships with healthcare providers and facilities to promote our programs.
Requirements
  • Associate Degree or Diploma in Nursing
  • New Mexico RN License
  • Minimum one year of acute care experience, with hospice experience preferred
  • Basic typing competency
  • Current driver's license and auto insurance
Preferred Qualifications
  • BLS certification
  • Hospice/Palliative Care experience
