Salary: $99,000 - $112,000

Albuquerque Health Care for the Homeless is committed to delivering comprehensive services to individuals facing homelessness since 1985. Our organization provides a unique continuum of integrated care through outreach and site-based services aimed at addressing the health-related factors associated with homelessness. We envision a society that is equitable and free from homelessness, believing that access to quality healthcare, affordable housing, and a living wage can resolve this issue.

We are seeking a full-time Nurse Practitioner who is dedicated to serving our client population. The ideal candidate will possess expertise in chronic disease management and substance use disorder treatment.

This role requires collaboration with various departments and agencies, necessitating a strong team-oriented mindset. We emphasize a holistic approach to well-being, and the candidate should be comfortable working in both clinic settings and as part of our mobile teams. Our treatment model prioritizes immediate care in outreach environments.

Understanding and compassion for the diverse challenges faced by those experiencing homelessness is essential. Many of our clients have underlying trauma, so familiarity with trauma-informed care is crucial. Our clinic operates on a shared client model, and the ideal candidate should be at ease collaborating with multiple practitioners.

We offer both walk-in hours and scheduled appointments, with no on-call or weekend responsibilities. Our team is deeply passionate about public health and the communities we serve.

Minimum Qualifications:

  • Master's degree in Nursing
  • CNP license through the NM Board of Nursing
  • Current DEA license
  • Current CSR license

Preferred Qualifications:

  • Two years of experience in a primary or family practice medical setting
  • Two years of experience in a Federally Qualified Health Center or public health environment
  • Experience managing co-morbid conditions
  • Familiarity with eCW EHR
  • Knowledge of harm reduction and trauma-informed care principles
  • Prior experience working with underserved populations or individuals experiencing homelessness
