Role Summary: This position is responsible for facilitating referrals for Hospice and Palliative Care from various sources, gathering and coordinating essential information to complete the clinical assessment of patients for the appropriate program and level of care within that program. The role encompasses shared accountability for both Palliative Care and Hospice referrals.

Qualifications

*Minimum of an Associate's Degree or Nursing Diploma required*Valid New Mexico RN License is mandatory. *At least one year of acute care experience is required, with hospice experience strongly preferred. *Basic typing skills are necessary. *Current driver's license and auto insurance are required.

**BLS certification is required upon hire; experience in Hospice/Palliative Care is preferred.
