Clinical Liaison

We are seeking a skilled Clinical Liaison to facilitate Hospice and Palliative Care referrals from all sources and obtain reviews & coordinates the necessary information to complete the clinical assessment of the patient for the correct program & correct Location within that program.
Key Responsibilities- Facilitate Hospice and Palliative Care referrals from all sources
- Obtain reviews & coordinates the necessary information to complete the clinical assessment of the patient
- Ensure accurate and timely completion of clinical assessments
- Collaborate with healthcare teams to ensure seamless patient care
- Associates Degree or Diploma in Nursing minimum
- New Mexico RN License required
- Minimum one year of acute care experience required and hospice experience strongly preferred
- Basic typing competency
- Current driver's license and auto insurance
- Hospice/Palliative Care Experience
