About the Role

We are seeking a skilled Clinical Liaison to facilitate Hospice and Palliative Care referrals from various sources. The ideal candidate will possess strong communication and organizational skills, with the ability to coordinate patient assessments and ensure seamless transitions between care settings.

Key Responsibilities
  • Facilitate Hospice and Palliative Care referrals from all sources, including healthcare providers and patients
  • Obtain reviews and coordinate necessary information to complete clinical assessments for patients
  • Ensure accurate placement of patients in the correct program and location
  • Assume dual accountability for Palliative Care and Hospice referrals
Requirements
  • Associate Degree or Diploma in Nursing
  • New Mexico RN License
  • Minimum one year of acute care experience, with hospice experience preferred
  • Basic typing competency
  • Current driver's license and auto insurance
Preferred Qualifications
  • BLS certification
  • Hospice/Palliative Care experience
