JOB SUMMARY:


The role is designed to further the mission of PATH by supporting individuals in achieving a more independent and fulfilling life as an integral team member.

The position involves collaborating with a team to deliver crisis intervention, support, safety planning, and connections to youth and families in Northeast Philadelphia, Region 1. As a Certified Peer Specialist, you will leverage your lived experience to provide support and perspective. The program operates year-round, 24 hours a day.


QUALIFICATIONS:
High School Diploma or GED.
Minimum of 1 year experience in the Behavioral Health system.
PA Peer Specialist Certification.
Valid Driver's License and personal vehicle required.
Proficiency in using an Electronic Health Record.
Strong interpersonal and communication skills.
COVID-19 vaccination, or willingness to receive the vaccine required.

SPECIFIC DUTIES:


Conduct crisis assessments, interventions, education, and support in person, alongside an assigned team member, within a specified timeframe for referrals received by DBH/CBH.

As a Certified Peer Specialist, share your lived experience to connect with and support individuals in crisis, assisting in the development of a crisis support plan.

Employ approved evidence-based practices and tools during crisis assessment and intervention, including SAMHSA crisis intervention guidelines and resolution models.
