Our Adult Residential Programs cater to adults facing challenges related to chemical dependency and mental health issues across various facilities in Albany County. The primary objective of these programs is to empower residents in managing their conditions and acquiring essential skills for successful community living.

Program Highlights:

  • Holt House: Offers 24/7 support for adults with mental health and developmental disabilities.
  • Apartment Program: Designed for adults ready for increased independence in one- or two-bedroom apartments.
  • Cohoes Residence: Provides round-the-clock assistance for adults managing mental health diagnoses.
  • Recovery Residence: Supports men with co-occurring mental health and addiction issues requiring continuous care.

Position Summary:

The Residential Case Manager plays a crucial role in developing service plans and managing cases for clients within the residential treatment environment.

Key Responsibilities:

  • Oversee resident activities within treatment facilities.
  • Assist clients in accessing community resources.
  • Deliver comprehensive treatment and case management services, ensuring compliance with OMH and Medicaid regulations.
  • Maintain a safe environment for all residents.
  • Evaluate clients' skills, strengths, and needs; instruct on essential life skills such as budgeting, hygiene, and socialization.
  • Participate in mandatory training sessions, including Crisis De-Escalation and CPR/First Aid.
  • Engage in quality assurance activities and compliance monitoring.
  • Document treatment service delivery, ensuring adherence to regulations (OMH, HIPAA, Medicaid, OMIG).
  • Develop and implement individualized transitional plans focused on client needs.
  • Enhance awareness and access to community resources.
  • Foster relationships among staff, peers, and family to promote self-sufficiency.
  • Assist clients in maintaining supplemental benefits provided by various agencies.
  • Perform additional tasks as assigned.

Physical Requirements:

  • Ability to stand for extended periods.
  • Capability to lift up to 20 pounds.
  • Valid driver's license and ability to operate a vehicle safely.

Qualifications:

  • Minimum: High school diploma with five years of experience in human services.
  • Preferred: Bachelor's degree in a related field or an Associate's degree with two years of relevant experience.

Work Schedule:

Shifts may vary based on program needs, including day, evening, weekend, and holiday availability.
