Key Responsibilities- Actively participate as a member of the Interdisciplinary Team (IDT).
- Coordinate the individualized Plan of Care for assigned clients; organize and lead IDT meetings, prepare necessary documentation, and ensure timely submissions.
- Communicate with families, correspondents, and staff regarding IDT meeting schedules. Develop Individual Staff Action Plans during semi-annual meetings.
- Address clinical issues promptly and effectively.
- Assist in training staff on the implementation of goals and services tailored for assigned individuals.
- Maintain regular contact with appointed guardians and family members.
- Evaluate and update service plans (DDP2, IPOP) as required, ensuring annual revisions are completed.
- Collaborate with care management to review Life Plan updates and verify their accuracy.
- Conduct site observations to ensure the effective implementation of service plans and adherence to guidelines, including dining and behavior plans.
- Oversee the preparation and execution of residential services.
- Advocate for assigned individuals by identifying valued outcomes and personal goals.
- Supervise service delivery in accordance with agency policies and relevant regulations. Be available for surveys as needed.
- Assist in the development of behavior plans and guidelines in conjunction with the agency's clinical department.
- A Bachelor's Degree in a human services-related field and at least one year of experience working with individuals with intellectual and developmental disabilities (QIDP status) along with a valid NYS driver’s license.
- Proven ability to function as a team leader, demonstrating strong verbal and written communication skills, and the capacity to coordinate comprehensive planning and implementation of individual program plans. Excellent organizational and problem-solving skills are essential.
