Our Adult Residential Programs cater to individuals dealing with substance use disorders and/or mental health challenges across various locations. The primary objective of our initiatives is to assist residents in managing their conditions while equipping them with essential skills for successful community living.
Program Details:
- Holt House: Supports adults with both mental health issues and developmental disabilities requiring round-the-clock assistance.
- Apartment Program: Designed for adults residing in one- or two-bedroom apartments who are prepared for increased independence.
- Cohoes Residence: Offers 24/7 support for adults with mental health diagnoses to help manage their symptoms.
- Recovery Residence: Focuses on men facing both mental health challenges and addiction, providing continuous support.
Position Summary:
The Residential Case Manager plays a crucial role in developing service plans and managing cases for clients within the residential treatment environment.
Key Responsibilities:
- Oversee resident activities within treatment facilities.
- Assist clients in accessing community resources.
- Deliver comprehensive treatment and case management services, ensuring compliance with regulatory standards.
- Maintain a safe environment for all residents.
- Evaluate clients' skills, strengths, and needs; provide training in functional skills such as budgeting, hygiene, and social interaction.
- Participate in mandatory and refresher training sessions.
- Engage in quality assurance activities to uphold compliance with residential program standards.
- Document treatment service delivery, ensuring adherence to regulations.
- Develop and implement individualized transitional plans for clients.
- Enhance awareness and accessibility of community resources.
- Foster relationships among staff, peers, and family members to promote self-sufficiency.
- Assist clients in maintaining their supplemental benefits.
- Perform additional tasks as assigned.
Physical Requirements:
- Ability to stand for extended periods.
- Ability to lift up to 20 pounds.
- Capability to drive a vehicle safely.
Qualifications:
- Minimum: High school diploma with five years of experience in human services.
- Preferred: Bachelor's degree in a human services-related field or an Associate's degree with two years of relevant experience.
Work Schedule:
Shifts may vary based on program needs, including day, evening, weekend, and holiday availability.
