Program Details:
- Holt House: Support for adults with mental illness and developmental disabilities requiring 24/7 assistance.
- Apartment Program: For adults ready for increased independence while living in one- or two-bedroom apartments.
- Cohoes Residence: Provides 24/7 support for adults managing mental health diagnoses.
- Recovery Residence: Focused on men with co-occurring mental health and addiction issues needing constant support.
Position Summary:
The Residential Case Manager plays a vital role in developing service plans and managing cases for clients within the residential treatment environment.
Key Responsibilities:
- Oversee resident activities within treatment facilities.
- Assist clients in accessing community resources.
- Deliver comprehensive treatment and case management services, ensuring compliance with relevant regulations.
- Maintain a safe environment for all residents.
- Evaluate clients' skills, strengths, and needs; provide guidance on functional skills such as budgeting and personal hygiene.
- Participate in mandatory and refresher training sessions.
- Engage in quality assurance activities to uphold program compliance.
- Document treatment service delivery, ensuring records meet regulatory standards.
- Develop and implement individualized transitional plans for clients.
- Enhance awareness and access to community resources.
- Foster relationships among staff, peers, and family members to promote self-sufficiency.
- Assist clients in maintaining their supplemental benefits.
- Perform additional tasks as assigned.
Physical Requirements:
- Ability to stand for extended periods.
- Capability to lift up to 20 pounds.
- Must be able to drive safely.
Qualifications:
- Minimum: High school diploma with five years of experience in human services.
- Preferred: Bachelor's degree in a related field or an Associate's degree with two years of relevant experience.
Work Schedule:
Shifts may vary based on program needs, including day, evening, weekend, and holiday availability.
