Job Overview

The Boston Public Health Commission is seeking a dedicated professional to assist clients within our Homeless Services emergency shelters, focusing on housing stabilization and retention.

KEY RESPONSIBILITIES:

  • Deliver follow-up housing stabilization services to individuals who have secured housing, which includes aiding with rent and utility payments, addressing landlord/tenant disputes, and connecting clients with community resources and support systems. Assist clients in acquiring furniture and facilitating their move into permanent housing.
  • Organize community integration initiatives for clients, imparting essential life skills and independent living capabilities necessary for thriving in stable housing environments.
  • Collaborate with community-based housing organizations to provide on-site housing search assistance. Conduct intakes and assessments for shelter guests to evaluate their needs and eligibility for various services and public benefits. Offer case management, crisis intervention, and supportive counseling to clients.
  • Provide transportation for guests to their housing-related appointments.
  • Facilitate case management, crisis intervention, and supportive counseling for clients. Coordinate service delivery and referrals in areas such as family reunification, accessing benefits, and legal assistance (including probation and court matters).
  • Maintain accurate and current service plan contacts and progress notes for departmental records.
  • Engage in a collaborative, multi-disciplinary team approach with other social service, rehabilitation, and health service departments within the shelter. Develop and lead budgeting, money management, and other life skills workshops. Participate in weekly treatment team meetings and additional assigned training sessions. Compile and maintain records and statistical reports.
