We strive to eliminate barriers and support the most vulnerable members of our community.
Position Overview:
We are seeking a detail-oriented and empathetic Patient Care Facilitator to join our One Care Team. This position entails working closely with healthcare teams to connect patients with vital services, offering administrative assistance, and actively engaging patients to enhance their overall health and well-being.
Key Responsibilities:
- Assist patients with complex health-related social challenges
- Conduct outreach to engage patients in services and support housing stability
- Maintain ongoing communication with the Managed Care Organization
- Help coordinate patient appointments and address various care needs
Required Qualifications:
- Bachelor's degree is preferred
- Bilingual abilities are highly desirable
- Experience in case management or related human services is preferred
- Excellent problem-solving and communication skills
