Job Summary

We are seeking a skilled and compassionate Home/Community Provider to join our team at Fairbanks Resource Agency Inc. As a Home/Community Provider, you will play a vital role in implementing habilitation plans for eligible participants, providing direct care, and promoting independence in their daily routines.

Key Responsibilities
  • Implement prescribed habilitation plans for eligible participants, providing routine direct care in the home, community, or center-based settings.
  • Work closely with families, guardians, and the FRA Home/Community Services Manager to design, implement, and modify intervention strategies.
  • Provide direct care services, including meal preparation, eating, toileting, bathing, dressing, and recreational activities, as per the Plan of Care (POC).
  • Assure health and safety needs are met, and prepare and/or assist individuals to prepare meals, ensuring nutritional needs are met.
  • Observe, supervise, and record participant activities, discussing responses to intervention strategies with the Home/Community Manager and other staff members.
  • Maintain records and prepare reports on services provided, and perform or request emergency aide as needed.
  • Stay current with knowledge of developmental disabilities and input participant information and case notes into the electronic medical record (Therap).
  • Attend training seminars, in-service sessions, and staff meetings as requested, and maintain training compliance in accordance with FRA policies and procedures.
Requirements
  • High School Diploma or equivalent.
  • Preferred six months experience working with individuals who experience developmental disabilities.
  • Must meet agency insurance criteria for operating agency and personal vehicles on agency business.
  • Demonstrate proficiency in direct care activities, effective communication, and observational and organizational skills.
  • Ability to work flexible schedules, independently, and prioritize activities with minimal direction.
  • Reliable transportation and a good driving record are required.
Working Conditions

This position involves medium work, exerting up to 50 pounds of force occasionally and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.

The worker is not substantially exposed to adverse environmental conditions.
